Can't Get You Out Of My Head
Question: A 32 year old obese female presents to you for evaluation of pulsatile tinnitus. Audiogram reveals mild bilateral SNHL. She notes that while she was on anti-hypertensive medication, the tinnitus seemed to decline mildly. Exam reveals normal external auditory canals and tympanic membranes. CT is obtained that demonstrates bilateral thinning of the tegmen mastoideum but no evidence of superior canal dehiscence. Empty sella is noted. What is the most likely diagnosis?
