Sequence Of Events

Question: A 14 year old male is evaluated for post-traumatic nasal obstruction. His mother notes that he was struck in the nose with a football about 1 year ago. Initially, he reported bilateral nasal obstruction with epistaxis that improved over the next several weeks. However, over the last 3 months he has noted progressive bilateral nasal obstruction. She has also observed a change in the shape of his nose that she attributed to puberty. Examination demonstrates poor nasal tip support, and collapse of the external nasal valve with inspiration. What is the most likely sequence of events that led to his present complaint?

a) Traumatic septal deflection with displacement off the maxillary crest

b) Displacement of the upper lateral cartilages at the keystone area

c) Fracture of the medial crura

d) Septal hematoma with resultant loss of caudal septum
