Eyes Peeled and Ears to the Ground

Question: A 42 year old female is referred to you for symptoms of vertigo, tinnitus, and hearing loss.  She was initially treated for suspected Meniere's with a low salt diet and diuretics.  She remarked that this provided her with no benefit.  Her office examination is normal with the exception of redness of the sclera bilaterally.  Caloric testing demonstrates bilateral vestibular hypofunction.  Her balance symptoms worsen to the point of oscillopsia on the current treatment.  What is the most likely diagnosis?    [Answer will be posted with next week's new question]
